The Bio-Emulation approach as written in structural analysis and visual synthesis defined the new bases to consider for replication of natural tooth structures with composite resin. The penta-laminar technique represents the ultimate implementation of this philosophy, with extraordinary results that resemble natural teeth in an extremely precise way. However, feasibility of this technique is compromised by it’s intrinsic complexity, not accesible to all clinical conditions. By analyzing the key factors of natural structure’s aging process and applying this knowledge to the material selection, it is possible to simplify techniques to make them approachable in all situations. From a bi-laminar technique to the penta_laminar technique we can learn to adapt our work in order to optimize the clinical outcome achieving cost-effective treatments to cover our patients needs and expectations.
Esthetic restorations that involve one single upper central incisor represent a great challenge for both dentists and technicians. Achieving optimal results with the minimum number of try-in tests or repetitions necessarily implies performing a systematic exhaustive shade analysis. Advanced photographic techniques need to be used for this task, including different kinds of illumination and precise calibration of RAW files. Restoration material and technique selection depends on diverse factors such as restoration extension, shade complexity, operator skills and the esthetic requirements on the long term. Ultimately, a successful treatment will always be the outcome of a good communication with patient and perfect teamwork between dentist and technician.

This presentation teaches us a possible way of problem solving with the use of different matrix systems for direct restorations, in posterior teeth. A clear strategy will be developped on how to adress the possible pitfalls when facing restoring difficult contact points.
The focus will be on creating optimal conditions for often challenging direct restorations. We will offer step by step protocols for deep cavities, shape and contour, different matrix systems and how to bring everything together to achieve more predictable results. Numerous clinical examples will help us to understand the basics but also the nuances that make the difference between success and failure.
Starting from the bio-mechanical behaviour of posterior teeth we come to the understanding of crack initiation, crack development and crack arrest. We will overview the early diagnosis of cracks, classification and workflow in addressing cracks following a bio-emulated approach. Clear guidelines will be presented and a clinical decision tree will be proposed to help the practician in his decision making. In this context we will try to answer the following questions: WHAT, WHY, HOW, HOW MUCH, HOW LONG.
And last but not least : how do we communicate with the patient about all this?
